Overview
Nerve pain (neuropathic pain) occurs when the nerves are damaged or not functioning correctly. At their best, the nerves send signals from different parts to the brain. Still, when irritated, compressed, or damaged, they can send faulty signals and confuse the brain into thinking the body is hurt, even when there is no injury in the tissues. This type of pain can be:-
- Peripheral: Nerve pains outside the brain and spinal cord (e.g., in arms, legs, feet).
- Central: Arising from the brain or spinal cord (for example, stroke conditions, multiple sclerosis, or injuries to the spinal cord).
Nerve pain is entirely different from other kinds of pain. Patients usually experience it as burning, stabbing, electric shock-like, or tingling. Sometimes, it may come and go, and most of the time, it will stay constant.
Causes of Nerve Pain
Nerve pain has so many causes:-
- Diabetes: damage done to the nerves by prolonged exposure to high blood sugar (diabetic neuropathy) usually involves the legs and hands.
- Trauma: Damage to nerves by stretching or cutting from accidents, fractures, or surgeries.
- Spinal Disorders: Herniated discs or spinal stenosis pressing on nerves.
- Shingles (Post-herpetic Neuralgia): Severe pain emanates from nerves when a person has been infected with shingles.
- Chemotherapy & Cancer: Some of the chemotherapeutic agents used for cancer treatment damage nerves, but tumors may also press on them.
- Trapped Nerves: Wrist if carpal tunnel syndrome is involved, lower back/leg if sciatica, pelvis associated with pudendal neuralgia.
- Neurological Conditions: such as multiple sclerosis, stroke, Parkinson's disease.
- Infections: HIV, syphilis, or Lyme disease.
- Alcoholism and Nutritional Deficiencies:Vitamin Deficiency Vitamin B12 damages the nerves.
Manifesting Symptoms of Nerve Pain
Most people with nerve pain would start to report:-
- Burning pain that doesn't improve with rest.
- Electric shocking staccato shots in arms, legs, or back.
- Pins and needles sensation.
- Radiating pain in penetrating degrees of sensory numbness or hypersensitivity (even light touch feels painful).
- Muscle weakness or poor coordination.
- Pain worsens at night and disturbs sleep.
- Radiating pain (e.g., sciatica running from lower back to leg). Unlike muscle pains, nerve pain doesn't respond, in general, to simple painkillers.
Diagnosis of Nerve Pain
Diagnosis begins at Dr Ekta's Pain Management Clinic, where we listen to your story and understand your symptoms. We then use sophisticated instruments to verify the nature of the cause:-
- Making a medical history and a physical exam to evaluate how the pain started and what aggravates/relieves it.
- Neurological Examination- Observation of Decrease in Sensation, Strength, and Reflexes.
- Conduction Studies (NCS) and Electromyography (EMG)- Measuring How Well Nerves and Muscles Are Working.
- MRI or CT scans provide information on structural causes such as disc problems or tumors.
- Blood tests will identify: diabetes, vitamin deficiencies, or infections.
- Diagnostic nerve blocks- Identify which nerve is responsible for the pain being produced.

- Radiofrequency Ablation (RFA): Use heat to turn off nerve fibers, transporting pain and providing prolonged comfort.
- Spinal Cord Stimulation (SCS): A Miniature device that changes the pain signals before reaching the brain.
- Peripheral Nerve Stimulation (PNS): Targeting nerves outside the spinal cord.
- PRP (Platelet-Rich Plasma) Therapy: Healing and regenerating nerve tissue using one's own growth factors.

Nerve Pain FAQ
Will Nerve Pain Ever Go Away?
Sometimes mild nerve pains can get better, but chronic ones usually require treatment.
How Does Nerve Pain Feel Different From Muscle Pain?
Nerve pain usually feels like a burning, tingling, hot sensation or an electric shock, while muscle pain is more like soreness or cramping.
Do Nerve Blocks Carry Risks?
Yes. They are minimally invasive, safe, and offer targeted blockade treatment.
Will Changes in lifestyle Help?
Yes. Moderate or light exercise, a balanced diet, and stress management can significantly alleviate symptoms.
Is there any indication for surgery?
Surgery for nerve pain is very rare. The only indication is if you have significant compression that has not improved with conservative management.
